This cheerful chap is showing us an instrument that was used to remove small bladder stones in the seventeenth century, before anaesthetics and antiseptics. Note the' blood-stained 'shirt!

Samuel Pepys was so pleased with his operation that he used his stone as a paperweight and held an annual 'Stone Feast' to celebrate. As his stone was large, he underwent an operation with a more fearsome instrument than the one pictured!
