Severe?

Had to complete an incident report today.
The intranet page that links to the form page makes the point that in the case of "severe death" we should telephone someone, but only after filling in the form...
What on earth constitutes "severe death"?
Is it like in the Omen, is impaled by a lightning rod a more severe death than being decapitated by a sheet of glass?
Is it where the employee dies and fails to fall over so no one notices.
Maybe the words 'injury or' should be between severe and death.
We may never know...
